I agree. I can think of a play and a non-fiction memoir-type book that fit this description. Was this a novel? A short story? Did you find it in a bookstore or check it out from a library?
Physical or mental torture? In Carol O'Connell's Judas Child there is a sub story about a psychologist being mentally tortured by his patient, by his describing the horrible crimes he commits. The psychologist can't say anything, because of patient confidence. The main story is about kidnapping and torturing children. Well written, but not for the faint of heart.
